RESUMEN

OBJECTIVE: To study the actual state of the organization of medical care for patients with the profile of «maxillofacial surgery¼ in the Southern Federal District of the Russian Federation in order to develop proposals for its optimization within the framework of a three-level system of medical care. MATERIAL AND METHODS: We analyzed accounting and reporting materials characterizing the organization of medical care in the field of «Oral and maxillofacial surgery¼ to the population in the Southern Federal District of the Russian Federation. RESULTS: The population of the Southern Federal District of the Russian Federation is currently provided with accessible round-the-clock medical care in the field of maxillofacial surgery. The forces and means at the disposal of the district government allow this to be implemented. However, the article describes a number of obstacles to the provision of high-quality medical care in the field of maxillofacial surgery and suggests ways to eliminate them. CONCLUSION: Preparation of an Order defining the functioning of a three-level system of medical care for adults and children with maxillofacial pathology in the Southern Federal District of the Russian Federation, as well as more effective use of telemedicine consultations will significantly improve the availability and quality of medical care.

Resumo O financiamento público vem ganhando destaque nos estudos das políticas públicas esportivas, pois permite visualizar de forma mais transparente onde e como são alocados os recursos públicos para o setor. Este artigo tem como objetivo analisar o financiamento das políticas públicas de esporte do Distrito Federal (DF), a partir de matriz e proposta metodológica que possibilitam o levantamento e sistematização de dados referentes à identificação das fontes, magnitude e direcionamento dos gastos públicos orçamentários com o esporte de 2009 a 2019. A pesquisa apontou: (i) falta de legislação efetiva para garantia dos recursos esportivos; (ii) centralidade da execução do orçamento pela Secretaria de Esporte do Distrito Federal; (iii) elevado gasto com a construção do Estádio Nacional de Brasília; e, (iv) hierarquização dos gastos esportivos nos programas oferecidos, com priorização aos Centros Olímpicos e Paralímpicos, ação com gestão terceirizada, além de capilaridade e abrangência reduzidas.


Abstract Public funding has been gaining prominence in studies of sports public policies as it provides a more transparent view of where and how public resources are allocated to the sector. This article aims to analyze the financing of public policies for sports in the Federal District (FD), based on a matrix and methodological proposal that allows the survey and systematization of data referring to the identification of sources, magnitude, and direction of public budget expenditure on sports from 2009 to 2019. The research points out: (i) the lack of effective legislation to guarantee sports resources; (ii) the centrality of budget execution by the Secretariat of Sports of the Federal District; (iii) high expenses with the construction of the Brasília National Stadium; and (iv) the prioritization of sports spending in the offered programs, with priority given to the Olympic and Paralympic Centers, outsourced management, in addition to reduced capillarity and scope.


Resumen La financiación pública ha ido cobrando protagonismo en los estudios de políticas públicas deportivas, ya que permite tener una visión más transparente de dónde y cómo se asignan los recursos públicos al sector. Este artículo tiene como objetivo analizar del financiamiento de las políticas públicas deportivas en el Distrito Federal (DF), a partir de una propuesta matricial y metodológica que permite recolectar y sistematizar datos referentes a la identificación de fuentes, magnitud y dirección del gasto público presupuestario en deportes, de 2009 a 2019. La investigación señaló: (i) la falta de una legislación efectiva para garantizar los recursos deportivos; (ii) la centralidad de la ejecución presupuestaria por parte de la Secretaría de Deportes del Distrito Federal; (iii) altos gastos con la construcción del Estadio Nacional de Brasilia; y (iv) la priorización del gasto deportivo en los programas ofrecidos, con prioridad en los Centros Olímpicos y Paralímpicos, gestión externalizada, además de capilaridad y alcance reducidos.

OBJECTIVE: The purpose of this research is to study the quantitative indicators of the provision of medical care using telemedicine technologies (TMT) with the remote interaction of medical workers with each other to obtain an objective assessment of the level of activity and involvement of medical organizations (MO) of different constituent entities of the Russian Federation (RF) in this process. MATERIAL AND METHODS: The study was carried out according to the reporting forms of the Federal State Budgetary Institution National Medical Research Center «Central Research Institute of Dental and Maxillofacial Surgery¼ (FSBI NMRC «CRID&MFS¼) of the Ministry of Health of Russia based on the results of councils of doctors using TMT. A total of 648 indicators were studied, reflecting the purpose of the consultation, the number of medical organizations that requested consultation in the specialty «maxillofacial surgery¼, the number of patients and requests. There were 685 applications for consultations in FSBI NMRC «CRID&MFS¼ of the Ministry of Health of Russia of 577 patients from 138 medical organizations of 72 constituent entities of the Russian Federation in 8 Federal Districts (FD) for 2021 processed. RESULTS: The most involved MO in the process of providing medical care using TMT with remote interaction of medical workers with each other are of the Ural, Central, and Volga Federal Districts, of which the most active are the Khanty-Mansky and Yamalo-Nenets Autonomous Districts (AD). The demand for clarification of the tactics of treatment by doctors of MO of the constituent entities of the Russian Federation is 1.8 times higher than in determining the possibility of hospitalization and 3.5 times higher than in clarification of the diagnosis. CONCLUSION: The indicators obtained as a result of the study can be included in the predictive analysis to determine the volume of TMT for the next calendar year, allowing in advance to rationally allocate personnel, time, information, material, and technical resources to ensure the process of providing medical care using TMT with remote interaction of workers with each other.

Phenolic acids are biologically active substances that prevent aging and age-related diseases, e.g., cancer, cardiovascular diseases, Alzheimer's disease, Parkinson's disease, etc. Cellular senescence is related to oxidative stress. The Siberian Federal District is rich in medicinal plants whose extracts contain phenolic acids. These plants can serve as raw materials for antiaging, antioxidant food supplements, and Amelanchier ovalis is one of them. In the present research, we tested the phytochemical profile of its extract for phenolic acids. Its geroprotective and antioxidant properties were studied both ex vivo and in vitro using Saccharomyces cerevisiae Y-564 as a model organism. The chromotographic analysis revealed gallic, p-hydroxybenzoic, and protocatechuic acids, as well as derivatives of chlorogenic and gallic acids. The research involved 0.25, 0.5, and 1.0 mg/mL extracts of Amelanchier ovalis, all of which increased the growth and lifespan of yeast cells. In addition, the extracts increased the survival rate of yeast under oxidative stress. An in vitro experiment also demonstrated the antioxidant potential of Amelanchier ovalis against ABTS radicals. Therefore, the Amelanchier ovalis berry extract proved to be an excellent source of phenolic acids and may be recommended as a raw material for use in antioxidant and geroprotective food supplements.

The aggressive infectious nature of SARS-CoV-2, its rapid spread, and the emergence of mutations necessitate investigation of factors contributing to differences in SARS-CoV-2 susceptibility and severity. The role of genetic variations in the human HLA continues to be studied in various populations in terms of both its effect on morbidity and clinical manifestation of illness. The study included 484 COVID-19 convalescents (northwest Russia residents of St. Petersburg). Cases in which the responsible strain was determined were divided in two subgroups: group 1 (n = 231) had illness caused by genovariants unrelated to variant of concern (VOC) strains; and group 2 (n = 80) had illness caused by the delta (B.1.617.2) VOC; and a control group (n = 1456). DNA typing (HLA-A, B, DRB1) was performed at the basic resolution level. HLA-A*02 was associated with protection against infection caused by non-VOC SARS-CoV-2 genetic variants only but not against infection caused by delta strains. HLA-A*03 was associated with protection against infection caused by delta strains; and allele groups associated with infection by delta strains were HLA-A*30, B*49, and B*57. Thus, in northwest Russia, HLA-A*02 was associated with protection against infection caused by non-VOC SARS-CoV-2 genetic variants but not against delta viral strains. HLA-A*03 was associated with a reduced risk of infection by delta SARS-CoV-2 strains. HLA-A*30, HLA-B*49, and HLA-B*57 allele groups were predisposing factors for infection by delta (B.1.617.2) strains.

The article presents the results of a comparative analysis of the provision of medical personnel. Differences in the indicators of provision with specialists of various profiles have been studied. It is noted that in the North Caucasian Federal District the total values ​​of the medical service density indicator are lower than in the Russian Federation by 2.1%. The highest rates of provision with doctors are registered in the republics of Ingushetia with an increase of 32%, and in the republic of North Ossetia. Higher values ​​of the provision indicator with the heads of the organization and their deputies were established in the North Caucasus Federal District than in the Russian Federation. At the same time, in most subjects of the North Caucasus Federal District there is a shortage of specialists in the following specialties: anesthesiologists-resuscitators, pediatric surgeons, neonatologists, pediatricians, psychiatrists, radiologists, epidemiologists. However, the indicator of provision with dentists in the North Caucasus Federal District exceeded the Russian one by 2 times. Based on the analysis, the authors present recommendations, including the need to eliminate the shortage of doctors in a number of specialties, increase the admission to residency in them, strengthen social support measures, temporarily restrict the admission of applicants in the specialty of dentistry, etc.

In the article, the authors identified the most attractive areas for investment in the Far Eastern Federal District based on statistical data on the agriculture and processing industry. The authors presented a set of secondary data from the Federal State Statistics Service and the Investment Projects Digital Platform, which reflect the cost of implementing projects in the Far Eastern Federal District. These data were presented to characterize the region as deeply diversified, with a predominance of the livestock and fishing sectors. The authors identified the most promising investment projects in various regions of the Far Eastern Federal District and assessed their attractiveness in terms of accessibility to transportation. The result was a map of the implementation of the main areas of investment activity, taking into account transport connectivity. In addition, recommendations have been drawn for the development of agriculture in the Far Eastern Federal District. These recommendations deserve the attention not only of government specialists but also of all stakeholders involved in agricultural production.

The increase in the number of older people (elderly and senile) among the population around the world is associated with a significant impact on the increase in the incidence of malignant neoplasms (MN). The federal project «Fight against oncological diseases for 2018-2024¼ has been launched in Russia within the framework of the national project «Healthcare¼. The article presents the results of a study of the general disability (primary and repeated) of the adult population due to MN in the Chechen Republic (2010-2020). The main trends in the dynamics of general disability (age differentiation, severity, level) in the Chechen Republic and in the North Caucasus Federal District as a whole are compared. Older people with disabilities prevail in the structure of both primary and repeated disability due to MN in the Chechen Republic. This corresponds to the indicators for the North Caucasus Federal District and correspond to the data for the Russian Federation as a whole. The level of disability in the Chechen Republic is lower compared to the indicators for the North Caucasus Federal District. The proportion of disabled people with severe group II and group I disabilities is higher in the republic compared to extensive indicators for the district. The formed structure of disability and the identified trends in its dynamics are a scientific and practical basis that is advisable to use when developing an integrated approach to the medical and social rehabilitation of disabled people and the prevention of health disorders due to MN, taking into account the identified features of disability of the population in the territorial subject of the Russian Federation.

Background: This article provides an assessment of the sustainability of Russian regions' financial systems. The study is based on the methods of generalization and synthesis, correlation-regression analysis, and multivariate classification. Since the structure of the regional financial system is complex, several works are devoted to studying its sustainability issues. The relevance of the study topic is confirmed by the lack of a systematic approach to assessing the integral index of sustainability and the possibility of using various tools in determining the complex indicator. Methods: This  methodology with application of mathematical statistics methods makes it possible to assess the financial system sustainability in four sectors, to include the leading indicators in the assessment, and to identify regions with extreme values of debt burden indicators. The method was tested for the regions of the Northwestern Federal District (NWFD) for the period 2010 - 2019  to classify the regions according to three levels of debt sustainability. Data collection from the 1 st January to 30 th April 2022 included statistical data from government open internet sources, sectors studied relate to government, and municipal budgets in the NWFD. Authors analyzed regional debt sustainability indicators and identified themes in the field of sustainability studies for the NWFD. Results: An increased level of financial system sustainability was observed among the  NWFD regions in the corporative and personal finance sectors, indicating a significant contribution of businesses and households to maintaining the balance and sustainability of the financial system in Russia as a whole. The results of the study also identified that the NWFD regions belong to three clusters: cluster 1 - high debt sustainability; cluster 2 - medium debt sustainability; and cluster 3 - low debt sustainability. Conclusions: The study results allowed the identification of regions with a constantly high level of debt, financial, and corporative sustainability.

The article provides a quantitative assessment of the socioeconomic potential of the territories of advanced development (TAD) of the regions of the Far Eastern Federal District (FEFD). The key indicators of the TAD of the Far Eastern Federal District are considered, their contribution to the dynamics of investments in fixed assets and employment of the regions of the Far Eastern Federal District is determined, groups of regions of the Far Eastern Federal District are identified according to the degree of influence of the TAD on the parameters of social, economic and investment development, the industry specialization of the TAD is analyzed, the contribution of the TAD to investment and employment is assessed in the economy of the Far Eastern Federal District until 2024.

The novel coronavirus disease 2019 (COVID-19) still challenges researchers due to its spread and deaths. Hence, the classical epidemic SIR and SEIRD models inspired by the epidemic's outbreak are widely used to predict the evolution of the disease. In addition to classical approaches, describing complex phenomena through Cellular Automata (CA) is a highly effective way to understand the iterations on a populated system. The present research analyzed the usage of CA to generate an epidemic-computational model from a micro perspective based on parameters obtained through a statistical fit from a macro perspective. After validating SIR and SEIRD models with the government official data for Brasilia, Brazil, the authors applied the obtained parameters to the Cellular Automata model. The CA model simulated the spread of the virus from infected to uninfected people in a restrained environment (i.e., a supermarket) under several varied conditions applying an approach never adopted before. The manner of applying CA in this research proved to represent an essential tool in predicting the spread of the coronavirus in confined spaces with random movements of people. The CA numerical open-source presented has the purpose of clarifying how the spread occurs not only as a mathematical curve but in an organic way. The numerical simulations from the CA model allowed the authors to conclude that markets and stores are relevant places where might be infections. Thus, every local store and the market owner should reason about the aspects that could avoid the spread of the disease, coming up with efficient solutions. Each environment has specific features that only those who know them are the ones capable of managing.

Os animais sinantrópicos, dentre eles os pombos, constituem uma fonte de incômodo e problemas de saúde pública no meio urbano. Encontram-se dispersos por todo o território brasileiro. Devido a sua domesticação e distribuição no ambiente, os pombos-domésticos (Columba livia) levam a queixas por nidificações, infestação por ácaros e notificações de doenças humanas. Diante disto, foi efetuado levantamento documental dos registros de reclamações feitas pela população à Diretoria de Vigilância Ambiental (DIVAL) do Distrito Federal, no período de 2014 a 2016, com o objetivo de mapear as áreas com vulnerabilidade e propor soluções para reduzir a presença desses animais sinantrópicos e a incidência de agravos. Alimento, abrigo e água são elementos essenciais para a aproximação e permanência com o homem. A distribuição das reclamações apresenta expansão principalmente na zona urbana, e outras onde nunca houve notificação, sendo que o número de notificações para estabelecimentos privados se mostrou maior que para públicos. O controle populacional desses animais é complexo e os serviços de controle encontram dificuldades em suas atividades.

Dispersedthroughout the Brazilian territory due to their domestication and distribution, domestic pigeons (Columba livia) of the Columbiformes class lead to complaints of mite infestation, nesting and notification of human diseases. In view of this, a documentary survey of the records of complaints made by the population to the Environmental Surveillance Directorate (DIVAL) of the Federal District was carried out, in the period from 2014 to 2016, in order to map the vulnerable areas and propose solutions to reduce the presence of these synanthropic animals and the incidence of injuries. The distribution of complaints is expanding mainly in the urban area, and others where there was never any notification, with the number of notifications for private establishments being higher than for public ones.The population control of these animals is complex and the control services face difficulties in their activities.

Agriculture in the Russian Federation is fundamental to the country's economic performance, living standards, the wellbeing of people and state safety. Considerations relating to food security, prospects of and challenges before plant breeding in the Siberian Federal District (SFD), the largest agricultural area of the Russian Federation, are provided in the article. The agricultural area used in the SFD is about 50 million hectares and accounts for 13 % of the country's gross grain production. The need for the introduction of modern molecular biological methods, bioengineering and IT technology is demonstrated and discussed. As Russia as a whole, Siberia is largely engaged in unpromising extensive farming practices, which rely on natural soil fertility, and this factor should be taken into account. Another issue is noncompliance with intensive farming technologies used for cultivating new-generation commercial cultivars. Although capital investments in plant breeding are the most cost effective investments in crop production, breeders' efforts remain underfunded. The article explains the need for fundamental reform in this economic sector: the recognition of plant breeding as being a fundamental science; a fair increase in its funding; the development of a breeding strategy, nationally and regionally; the further expansion of the network of the Breeding Centers; the re-establishment and improvement of the universities' departments specialized in plant breeding and seed production; having more state-funded places in the universities for training plant breeders to be able to maintain and cement the country's advanced position in plant breeding and to develop new globally competitive next-generation cultivars of main crops. Should these issues be ignored, all the problems that have accumulated to date will lead to risks of long-term instability in this economic sector. The need for the careful preservation of continuity in plant breeders and plants being bred is stated. The regulatory functions of the state and agricultural science in plant breeding, plant industry and seed production are considered.

The article reflects the position held by the Institute of Economics and Industrial Engineering, Siberian Branch of the Russian Academy of Sciences regarding the problems and prospects of the socio-economic, scientific, and technological development of Siberia and the directions of strengthening its positions in the world and Russian space. The main development problems of Siberia and the Siberian Federal District are identified, taking into account modern challenges and threats. It is shown that the recovery of the Siberian economy as a whole and the regions of the Siberian Federal District, in particular, should be based on a significant increase in investment activity at all levels: macroregional, regional, and municipal. Proposals are made on a package of new Siberian investment projects with a focus on "embedding" Siberia in high-tech niches of the Russian and world economies, as well as on a system of priority measures that should be taken at the national, interregional, and regional levels.

This article analyzes the main indicators of the incidence and life expectancy of the population of the Republic of Sakha (Yakutia) over a twenty-year period. An increase in the incidence rate of the population was revealed that exceeded the average for the country and the Far Eastern Federal District, including for oncological diseases, the level of which increased several times in the republic. The value of the indicator of life expectancy in the republic approached the average Russian level, while 20 years ago a much lower indicator was determined. In the dynamics of indicators of the morbidity of the population and life expectancy over a twenty-year period, a polynomial trend has been outlined. For example, the incidence of neoplasms and diseases of the circulatory system showed a sharp increase in 2004, followed by a decrease, for diseases of the digestive system - similarly, an increase in 2001 and a subsequent decrease. This aspect requires an additional detailed study of the causes of such changes in order to be able to predict them in the future.

RESUMEN

The article presents the results of analysis of values and dynamics of changes in indicators of fertility rate, expected life expectancy, total and infant mortality, able-bodied mortality, mortality of circulatory system diseases, neoplasms, and road traffic accidents in 18 subjects of the Central Federal Okrug of the Russian Federation in 2015-2018. The results of comparative analysis of factual values of mentioned indicators with values of target indicators approved by the "Health care Development Strategy in the Russian Federation for period up to 2025" (2019) are presented. The necessary scope of correction of existing discrepancies are established. Reducing by 20% indicators of mortality of able-bodied age, circulatory system diseases and road accidents is needed to reach target values in more than half of the subjects of the Okrug for each cause of death. Based on the results of comprehensive ranking of analyzed health indicators, rating position of each subject in the Central Federal Okrug is determined. For the subjects of the Central Federal District, the analysis established significant differences in indicators values, volume of their changes, possibilities of achieving target goals, rating position and inter-territorial gaps. It is demonstrated that differences in extreme values of summing of ranks of the subjects of the Central Federal District increased up to 5.1 times in 2015, up to 5.5 times in 2016, up to 6.0 times in 2017 and up to 6.2 times in 2018 that substantiate marked tendency of increasing territorial differentiation.

The completely formed database of the Population Cancer Registry (PCR database) of the North-Western Federal district of the Russian Federation in February 2019 has significantly expanded the ability to objectively assess the activities of the cancer service on a more reliable basis of analytical materials. In Russia, for the first time, it was possible to conduct epidemiological studies of malignant tumors for rare tumor locations, any age groups, and calculations of patient survival rates. The PCR database has more than 1 million observations and includes all the headings and subheadings of ICD-10 for class II - codes C00-96. The article will present the dynamics of morbidity and mortality of the population of the northwestern Federal district of the Russian Federation in comparison with the national average, the reliability of accounting and survival of patients with malignant tumors. For 8 years, the growth of cancer incidence in the Northwestern Federal district of the Russian Federation by 10,4% in standardized indicators and a decrease in population mortality in the Northwestern Federal district of the Russian Federation by 11,4% in the same indicators. The dynamics of the 5-year observed survival rate of patients with Northwestern Federal district of the Russian Federation, taking into account age groups, is calculated. The reliability of the PCR database of the Northwestern Federal district of the Russian Federation is confirmed by the fact that the materials of 3 administrative territories are included in the IARC monograph «Cancer incidence in five continents. Vol. XI¼ (Saint-Petersburg, Arkhangelsk region and the Republic of Karelia).

BACKGROUND: Diseases characterized by elevated blood pressure are the most frequently diagnosed pathology among socially significant diseases in the Russian Federation. According to clinical recommendations of the Russian Medical Society for Arterial Hypertension 2010-2019, the use of two and more component drugs improves the compliance of patients to treatment. The purpose of this study was a comparative analysis of preferences in the usage of combined antihypertensive drugs in ten regions of the Far Eastern Federal District (FSD). MATERIALS AND METHODS: Intra-group analysis of data on the sale of antihypertensive drugs in pharmacy organizations (162 drugstores) for the period 2018 was carried out in the 10 regions of the FSD. The sample is uneven due to the difference in total number of drugstores in the regions. The sample was random. Pharmacies were in urban and rural areas. The total purchase of all defined daily doses of all antihypertensive drugs was taken as 100%. Then, fixed-dose combinations were selected. The share of each drug in the sales structure was then calculated. Kruskal-Wallis criterion was used for determining the differences in the structure of acquisition of drugs between the regions. The Spearman's rank correlation and factor analysis were used for determining the correlation between the drug's acquisition structure and the FSD region. RESULTS: Two-component drugs are most in demand in the Amur region - 15.2% and Zabaykalsky Krai - 12.3%, and the lowest demand in the Sakhalin region - 5.25% and Magadan region - 6.5%. The largest sales indicators of three-component drugs were observed in the Zabaykalsky Krai - 1.2% and the Republic of Buryatia - 1.0%. The most acquired combinations were atenolol+chlorthalidone, valsartan+amlodipine, losartan+hydrochlorothiazide, perindopril/lisinopril/ramipril+amlodipine, perindopril+indapamide. CONCLUSION: Group leaders may vary depending on the region, which is due to the preferences of medical specialists. However, the differences are not significant, which proves the unity of approaches to combined drug therapy of arterial hypertension.

The present study examines the characteristics of registration accuracy and efficiency of treatment of patients at the population level using reliable analytical indicators of survival of patients, calculated according to international standards, taking into account the histological structure of tumors and the stage of the disease. For any research development, we must first ensure the reliability of the material selected for research. Among the six main most reliable criteria for evaluating the activities of the cancer service, such as morbidity, mortality, and the level of morphological verification of tumors, such indicators as the index of reliability of accounting, primary mortality, and survival of patients with malignant neoplasms are of the most important importance. The aim of the work is to determine the level of reliability of accounting for patients with skin melanoma (C43) and its dynamics from 1995 to 2018. for Russia as a whole and for a number of administrative territories. Calculate the five-year survival rate in the North-Western Federal District, taking into account the histological structure of tumors and the stage of the disease. The material for the study is statistical collections, including author's ones, the database of the cancer register of the North-Western Federal District of the Russian Federation. Standard methods of statistical analysis were used. It is important to note that for all administrative territories whose population cancer registry databases were used to calculate the five-year observed and relative survival rates, the survival rate of patients with malignant skin melanoma is significantly lower than the European average (Eurocare-4). Thus, the study revealed a significant improvement in the treatment of patients with malignant melanoma of the skin. At the same time, the estimated five-year observed and relative survival rates of patients with malignant melanoma of the skin remain relatively low.
